The Supported Living Services (SLS) Waiver is a pivotal program for adults aged 18 and above with Intellectual or Developmental Disabilities (I/DD). This waiver is designed to provide the essential services and supports needed for individuals to live comfortably and independently in their own homes and communities.
Personalized Support for Each Individual: Every adult under the SLS Waiver is evaluated using the Supports Intensity Scale (SIS) to identify their unique needs. This comprehensive assessment, conducted in collaboration with your Case Management Agency (CMA), covers various aspects including:
- Individual Basic Support Needs
- Medical Support Needs
- Behavioral Support Needs
Tailored Support Levels and Funding: Following the assessment, your adult family member will be assigned a specific support level, determining the funding they receive to cater to their ongoing needs. The support levels and corresponding funding are as follows:
- Level 1: $20,798.87
- Level 2: $27,778.32
- Level 3: $31,236.98
- Level 4: $35,885.54
- Level 5: $43,237.91
- Level 6: $56,671.69
